Mental health care encompasses more than therapy appointments and medication. For individuals struggling with behavioral health challenges, everyday tasks can become unmanageable. It can be difficult to meet basic needs, such as food, housing, healthcare, employment, and vocational training.
Mental Health Case Managers help individuals navigate an array of treatment options, behavioral healthcare programs, and community resources. Case Managers coordinate care to make sure individuals receive the appropriate treatment and customized care. They also connect individuals with available community resources, such as Social Security benefits, housing assistance, vocational training, employment, and healthcare.
Case Managers monitor treatment to help individuals on their path to mental wellness. They review treatment plans and assess goals to make sure individuals receive the highest quality of services.
Case Managers are based at RACSB’s five outpatient clinics, Rappahannock Regional Jail, Rappahannock Juvenile Detention Center, and Micah Ecumenical Ministries’ Hospitality Center.
