Home is often the best place to heal for individuals with a severe and persistent mental illness. Too often, they end up in jail or on the streets. Our Program for Assertive Community Treatment aims to keep people with severe and persistent mental illness at home. The PACT team meets individuals where they are and works with them around-the-clock. These intensive services help individuals connect to the resources they need to avoid homelessness, incarceration, and hospitalization.

The team includes mental health specialists, a psychiatrist,   nurses,  clinicians, substance and vocational specialists, and a peer specialist. This team provides care coordination and behavioral healthcare–and a whole lot more. Some services include: managing medications, keeping appointments, finding a job, finding affordable housing, and learning daily living skills.
