Our support coordinators work help individuals with a developmental disability navigate services. Support coordinators assess individuals to determine their needs, then find the most appropriate services.

The support coordinator serves as the primary contact to help navigate the Developmental Disability waiver process, including identifying needs and connecting individuals to services.

A support coordinator helps develop a personalized plan for each individual, then follows up to make sure the plan is working. Support coordinators also connect individuals with developmental disability to other community services, such as Social Security benefits, housing, and healthcare.

Through support coordination, individuals with developmental disability are able to live full, productive lives in our community.

To schedule a screening for a Developmental Disability waiver, a written diagnosis is required. This must be a signed diagnosis from a licensed professional working within their scope of practice. You may call 540-479-8568 and select option 1 to set up a screening or email the required document to [email protected] to expedite the process. Screenings typically occur at our CSB location and take approximately 2-4 hours. The individual being assessed must be present.  A list of required documentation will be emailed to you ahead of time.
